Non-Resident Enrollment Requirements
- School of Choice Application
- Suspension/Expulsion Policy
- Probation Form
- District Enrollment Form
- Birth Certificate
- Immunization Records
- Emergency Card
- Transcripts (9th-12th grades) / Report Cards (K-8th grades)
- Proof of Residency in (Wayne, Macomb, Monroe, Oakland, and Wasthenaw Counties) - (1 piece, a Current Valid Driver's license or State ID)
- Physical
- I.E.P., M.E.T. & Reports for Special Education (if applicable)
- Lunch Application (Only 1 application per family)
- Parent Connect Form (Only 1 application per family)
Immunization Requirements
- DTP - 4 doses - Last dose no later than 5 years ago. (4th dose must be on/after 4th birthday)
- Polio - 3 doses (3rd dose must be on/after 4th birthday)
- MMR (Measles, Mumps, Rubella) - 2 doses
- Meningococcal Vaccine - (Required for Children 11 to 18 years)
- Hepatitis B Series - (3 shots needed but series must be started)
- Varicella - 2 doses (or must have had Chicken Pox)
